ABSTRACT

Objective To develop a MOSAIQ Integration PlatformCHN (MIP) based on the workflow of radiotherapy (RT) and to meet the actual requirements in China and the special needs for the radiotherapy department. Methods MIP used C/S (client-server) structure mode running on the local network in the hospital and its database was based on the Treatment Planning System (TPS) and MOSAIQ database.Five network servers,as the core hardware, supplied data storage and network service based on cloud services. The core software was developed based on Microsoft Visual Studio Platform using C# network programming language. The MIP server could simultaneously offer network service for about 200 workstations, including entry, query, statistics, and print of data. Results MIP had 15 core function modules, such as Notice,Appointment,Billing,Document Management (application/execution),and System Management, which almost covered the whole workflow of radiotherapy.Up to June 2016,the recorded data in the MIP were as follows:13546 patients,13533 plan application forms,15475 RT records,14656 RT summaries,567048 billing records, and 506612 workload records. Conclusions The MIP based on the RT workflow has been successfully developed and used in clinical practice. It is an important part of radiotherapy information system construction with the advantages of intuitive operation,real-time performance, data security, and stable operation. It is digital,paperless,user-friendly,and convenient for the retrieval and statistics of data as well as information sharing and department management, and can significantly improve the efficiency of the department.More functions can be added or modified to enhance its potentials in research and clinical practice.

ABSTRACT

Objective To explore the clinical efficacy of one-stage cannulated screw fixation in treatment of tibial avulsion fracture at the insertion of posterior cruciate ligament(PCL) under posterior direction version and in treatment of tibial avulsion fracture at the anterior cruciate ligament (ACL)under arthroscopy. Methods From January 2010 to June 2015,25 patients with tibial avulsion fractures of the posterior cruciate ligament combined with the anterior cruciate ligament in our hospital were retrospectively analyzed.Firstly, arthroscopic exploration was conducted in all patients in their horizontal position, aiming to observe whether combined injures existed or not and to confirm fracture fragments and the degree of tibial avulsion fractures of the cruciate ligaments. All patients recieved arthroscopic fragment fixation by using cannulated screws, then, were treated with a minimally posteromedial incision in the prone position by cannulated screw fixation. All knee joints were protected by adjustable orthopedic brace for 4-6 weeks. The clinical knee joint function of patients were measured by using International Knee Documentation Committee (IKDC) scoring scale and Lysholm scoring scale. The X-ray or CT image were used to evaluate healing process of the fractures. Results All patients were followed up for 6-24 months, the median follow-up time was 15 months. All fractures healed in one stage, time of fracture healing ranged from 9 to 24 weeks, and the median time was 12 weeks. The average IKDC scores at the finial follow-up were as follows:claudication (4.88±0.29)points,support (4.65±0.36)points,pain (4.78±0.49)points,swelling (4.88±0.52)points,squat (4.85±0.35)points,all were higher than those before operation, there were statistically significant differences when compared with those assessed before operation (P0.05).Conclusion The one-stage cannulated screw fixation in treatment of tibial avulsion fracture at the insertion of PCL under posterior direction version and in treatment of tibial avulsion fractures at the ACL under arthroscopy is easy and convenient to operate, which has the advantages of reliable fixation and satisfactory recent curative efficacy.

ABSTRACT

Objective To explore the nursing method for the prevention and treatment of vaginal adhesions in patients treated with full-dose radiotherapy for cervical cancer. Methods From 2015 to 2016, 80 cervical cancer patients who received radiotherapy were treated with domestic vaginal washing, vaginal local use of"Fufangbao", infection control, diet and sexual rehabilitation guidance, and so on, and the nursing outcome was evaluated. Results Of the 80 patients, 6 were lost during follow-up, resulting in a follow-up rate of 925%. Ten, five, and three patients were found to have grade 1, 2, and 3 vaginal toxicity, respectively. The patients with grade 1 vaginal toxicity were not treated with special treatment and were asked to do more vaginal washing. The patients with grade 2 and 3 vaginal toxicity were treated with daily vaginal washing and"Fufangbao" for repairing the vagina;vaginal adhesions disappeared or improved 3 months later. The patients with no vaginal adhesions had regular sexual intercourse and thus had improved quality of life. Conclusions Effective clinical nursing can reduce the incidence of vaginal adhesions and stenosis, improve patients' quality of life, and promote the harmony of family.

ABSTRACT

Objective We observed the effect of topical application of mucopolyccharide polysulfate cream (hirudoid) on phlebitis prevention initiated by continuous infusion of 5-Fu with Baxter infusor. Methods 60 untreated patients with nasopharyngeal carcinoma were selected and randomly divided into the control group (28 patients) and the experimental group (32 patients). The patients of the experimental group were gently massaged with hirudoid along the infusion vein since the beginning of 5-Fu infusion, while the control group received no intervention. The incidence of phlebitis was observed in the two groups. Results The incidence of phlebitis in the experimental group was lower and the degree was lighter compared with the control group. Conclusions Hirudoid can decrease the incidence and released the symptoms of phlebitis initiated by continuous infusion of 5-Fu with Baxter infusor, and is effective to prevent and treat phlebitis.

ABSTRACT

Objective To explore a simple and effective operative procedure for treatment of pa-tellar fractures. Methods The clinical data of 108 patients (including 76 males and 32 females) with patellar fractures were analyzed. The age of the patients ranged from 18 years to 82 years. There were 67 patellar fractures on the right extremities and 41 on the left.Fracture types included transfractures in 43 patients, comminuted fractures in 54, torn fractures in eight and longitudinal fractures in three.Period from injury to operation ranged from 3 hours to 10 days.During operation, the broken patella was exposed for reduction and temporary pliers fixation;then, a five-pointed star woven with two absorbable sutures was placed on the broken patella, two semi-circular sutures around the patellar edge were made with su-tures which were through five points of the five-pointed star. When two sutures were pulled and knotted, the five-pointed star was also stretched to fix the patellar fractures firmly.Models of transverse patellar fractures were made in 20 knee joints of catties, which were divided into two groups randomly.Patellar fractures in Group A were fixed with five-pointed star lattice sutures and those in Group B with AO inten-sion bands.Loading test was performed on quadriceps femoris with materials test system for measuring the width of each fractured patella after the test. Results All patients were followed up for 6-60 months (mean 20 months) , which showed that all patellar fractures were healed.According to Bostman scoring system, the efficacy was excellent in 76 patients and good in 32.The experiment showed no statistical difference in the fracture disjunction distance between two methods (P > 0.05).Conclusion For treatment of patellar fractures, five-pointed star lattice sutures have the advantages of simple operation, reliable fixation, early postoperative exercise, fast recovery, satisfactory outcome and free need of reoper-ation for removing internal fixation.
